Our staff will require the following information:

  • Company or School name, contact, address, phone and fax number
  • Ship-to address, if different than above
  • Email contact
  • Requested in-hands date
  • Item number, item description
  • Quantity desired
  • Cushion color and imprint color
  • Placement of logo
  • Size of imprint
  • Method of payment. Acceptable forms of payment are cashier's check, money order, Visa, MasterCard or American Express.

Pending receipt of your order, we will fax or email an order confirmation for your approval. This order form requires a signature and payment information.

Pending receipt of your order approval, we will begin production of your art proof.  Your art proof will be sent by email. 

100% of your order total is due at time of art approval.  For Net 30 day terms, complete our credit application.
